  • Adjunct faculty and students: Please complete the blue portion of this form (adjunct faculty requests require Dean's Office approval).

  • PSR events only: Special Room Setup Requests need to be made separately on a maintenance request form.
    Please Note: Work request must be submitted 1 week in advance of the event. If setup and breakdown time is not scheduled event planners will be responsible for both setup and breakdown!
